In order for Bolle to process your single-vision lens order for Rx frames, please provide the following information by Fax (see foot of page) or Email:

• Your Name
• A certified copy of your Optical Prescription (dated less than 2 years ago) including the Pupillary Distance (PD)
• The Frame Model, the Frame Colour and the Lens Colour required

Please note:

1. Bolle point out that a short adaptation period may occur for people who have not previously worn a wrapped frame. In some rare instances, non-tolerance may occur.

2. Prescription Sunglasses are not included under our returns policy.
